Clinical Care Coordination
The RN performs assessments of individuals’ overall health status, acts as a liaison between Metro and outside medical providers regarding coordination of medical care. The RN provides disease and medication education/adherence, lab interpretation education for clients and nutritional counseling.
The RN acts as an additional medical support/resource on a client’s treatment team as well provides educational seminars on various health and wellness topics to both employees and/or client support groups.
Clinical Care Coordination provides these services to individuals with HIV+/AIDS Diagnosis and present with heightened medical needs (co-diagnosis, newly diagnosed, etc) and HIV- Negative Individuals who are At-Risk and need medical education.
