Medical Case Manager (Service Integration-Recovery Now, Tampa/St. Pete)
Metro Inclusive Health (METRO) is seeking a Medical Case Manager to provide coordination of services for all aspects of care and provides HIV and Hepatitis prevention services (e.g. HIV & HCV testing, risk reduction counseling & education, referrals to pre-exposure prophylaxis (PrEP) and Hepatitis A/B vaccination) within a behavioral health care setting. The goal of the Medical Case Manager is to aid in the reduction of co-occurring epidemics of HIV, Hepatitis, substance use disorders, and mental health conditions by helping clients access resources and evidence based, culturally appropriate behavioral health services and substance use treatment that is integrated with HIV prevention and medical care services.
This full-time position is based out of METRO’s Tampa offices with intermittent travel to METRO’s St. Petersburg offices. This position has a primary focus on serving racial and ethnic minorities; gay, bisexual, and other men who have sex with men (MSM); people who inject drugs (PWID); and transgender adults (ages 18+) within Hillsborough and Pinellas Counties.

Key Responsibilities Include:
- Conducting comprehensive needs assessments to develop individualized client service plans.
- Coordinating and assisting clients with accessing programs & services, including navigation and follow-up of establishing behavioral health services, medical services, food and transportation referrals, emergency financial assistance, and more.
- Provide insurance and benefits navigation, assisting clients with accessing public and private programs for which they may be eligible (e.g., Medicaid, Medicare Part D, State Pharmacy Assistance Programs, Pharmaceutical Manufacturer’s Patient Assistance Programs, other state or local health care and supportive services, and insurance plans through the health insurance Marketplaces/Exchanges).
- Continuously monitor clients’ progress to assess utilization of services and efficacy of the service plan.
- Re-evaluate client service plans every 6 months, with adaptations as necessary.
- Utilize evidence-based practices for individuals with mental health disorders or co-occurring disorders that are trauma informed, recovery oriented, and culturally appropriate.
- Work collaboratively within an interdisciplinary team (e.g. medical providers, behavioral health therapists, other agency personnel, community partners, members of the client’s circle of care) to provide counseling support, education, skill building, and navigation to support services to promote adherence to medical & health services.
- Conduct visits with clients in varying settings including in the community and the client’s home.
- Provide HIV, hepatitis C, and pregnancy testing and counseling.
- Refer and link individuals to HIV and hepatitis prevention and treatment services including but not limited to: PrEP/nPEP; hepatitis A & B vaccination; hepatitis C treatment; STI/STD testing & treatment; and HIV medical care.
- Develop and maintain key skills, competencies and knowledge of local/state/federal community resources, and HIV/AIDS, STI/STD, and hepatitis education and prevention.
The successful candidate will…
- Be able to speak, read, and write proficiently in English. Bilingual Spanish highly preferred.
- Have a bachelor’s degree in a Human Services related field.
- Have a minimum of one year’s work experience within a Human Services related field.
- Have knowledge of HIV/AIDS.
- Have experience and work comfortably with diverse, underserved, and at-risk populations.
- Have awareness and sensitivity to cultural differences, varying sex/sexuality/sexual orientations, relevant life experiences with substance use, HIV/AIDS, PrEP/nPEP, STI’s, and psychosocial issues concerning the HIV epidemic.
- Possess excellent active listening, verbal communication, de-escalation, and problem-solving skills.
- Possess the ability to effectively engage, interact and collaborate with team members, community members, and community collaborators.
- Demonstrate good organizational skills, multi-tasking skills, and the ability to meet assigned objectives and deadlines.
- Be detail oriented for accurate and thorough documentation of activities through case notes, assessments, and other program forms.
- Be able to work independently, in varying settings specific to position.
- Have a personal vehicle, valid Driver’s License and vehicle insurance.
- Be able to commute between Tampa and St. Petersburg locations.
- Be willing to submit to, and able to pass, a Level II background check.
- Complete required case management and HIV testing counselor trainings, maintaining certifications as required by, but not limited to the Grantee/Lead Agency, Health Resources and Services Administration, the Florida Department of Health, Bureau of HIV/AIDS Community Programs Unit, and the Certified Application Counselor Program for the Federally Facilitated Marketplace.
